Need to find a provider quickly? Our online provider search gives you flexibility in a simple format. We update the online search weekly. Just click below on the provider search link in the blue box.
Always verify that the provider you choose is participating in our network before you receive services. While it is our goal to provide accurate information, our provider network is subject to change without notice. Providers in all areas of our service region are included in the provider search. Please be sure to select the correct product (PPO, etc.) when beginning the provider search.

- MHNet Behavioral Health Providers
- Coventry Health Care of Illinois, Inc. ("Coventry Health Care") partners with MHNet to deliver behavioral health benefits to our members. Most members must have these services pre-authorized before accessing them.
- Freedom PPO and Optimum PPO Provider Search

Coventry Health Care Freedom PPO and Optimum PPO Level 3 includes Coventry National Network providers. To find Level 3 providers, click this link and select Freedom / Optimum Level 3 under the product drop down field. Level 1 and 2 providers can also be found by using this link.
Please note: Coventry National Network providers may not be available in all areas.
- Bariatric Surgery Centers of Excellence
- All members with bariatric surgery benefits who meet the applicable criteria for weight management surgery must have these services performed at one of our Bariatric Surgery Centers of Excellence in order for the services to be covered. These services require prior authorization from Coventry Health Care and the ordering provider. Click here to view a list of Centers of Excellence.
- EyeMed Vision Providers
- Some Coventry Health Care plans include access to EyeMed Vision providers. To find out if your benefit plan includes EyeMed, please check with your Benefits Administrator or call Customer Service.
- Heartland Dental Providers
- Some Coventry Healh Care plans include access to Heartland Dental's network. To find out if your benefit plan includes Heartland Dental, please check with your Benefits Administrator or call Customer Service.
- QualSight LASIK Providers

Out-of-Area Providers: PPO Members
PPO members on an out-of-area plan and living outside the service area use the Coventry Health Care National Network. Dependents living outside the PersonalCare service area and enrolled in the Passport program may also access the Coventry National Network for in-network benefits. Please click on the link below to search for a provider. After choosing your desired provider type, please select Coventry National Network from the drop-down menu.

Coventry Health Care National Network providers are available when you need urgent or emergency care outside the service area.
